1. Participation in such activities and use of equipment can result in personal harm, loss, damage, injury, or death. I accept these risks on behalf of myself and my heirs. I waive and release any potential future claim against The Southern Illinois Coalition for the Homeless, the City of Carbondale, and the Center for empowerment and justice that may in some manner be responsible for sponsoring, directing, teaching, or supervising my activities, the activities of others, or that own, control, or maintain the physical location in which such activities or programs are facilitated. The City of Carbondale does not warrant, or guaranty that the physical area of any Activity is safe and appropriate for such an activity and/or that any equipment used is in right and proper working order. It is my sole and personal obligation to ensure that the site and material are safe and appropriate for my use.

2. I am responsible for determining whether I am physically and medically able to participate in any volunteerism. I am responsible for determining whether a physical or medical examination should be undertaken before I participate in volunteerism and I will abide by any determination, limitation, or recommendation that may be issued by my medical or health care provider. Before, during, and after an Activity, I am solely responsible for determining my health and physical status and whether I can or should discontinue my participation in the Carbondale Warming Center, or take other actions, to protect my health or safety. The Southern Illinois Coalition for the Homeless and City of Carbondale assumes no duty to me to ensure my physical or medical ability to participate in these activities, whether before, during, or after such Activities.

3. I will conduct myself safely and reasonably. I will not participate in any volunteerism while under the influence of medication, drugs or alcohol, or while suffering from any medical, health or other condition (including, but not limited to, illness or injury), that might in some manner potentially cause harm or damage to me, to another participant, or to the property of the City of Carbondale. I will defend and indemnify The Southern Illinois Coalition for the Homeless and City of Carbondale, and its agents and employees, in response to any claim for injury, damage, or loss arising in some manner from my participation in any volunteerism.

4. Permission to Use Photograph, for all activities related to volunteerism at the Carbondale Warming Center I grant to The Southern Illinois Coalition for the Homeless and City of Carbondale, its representatives and employees the right to take photographs of me and my property in connection with the above-identified subject. I authorize The Southern Illinois Coalition for the Homeless and City of Carbondale, its assigns and transferees to copyright, use and publish the same in print and/or electronically. I agree that the City of Carbondale may use such photographs of me with or without my name and for any lawful purpose, including for example such purposes as publicity, illustration, advertising, and Web content.

5. Permission to Run Background, I hereby authorize the City of Carbondale to conduct a security background check on me. I understand that this security check will cover information including, but not limited to, criminal history, education, and employment. I hereby release the City of Carbondale and its elected officials, employees, agents and assigns, from all liability resulting from the furnishing of this information to the City of Carbondale.

6. ALL applicants Please note that any position offered through the Carbondale warming center is temporary and seasonal. Generally, the center will only operate in the winter months of the year December to March unless unforeseen Circumstances demand otherwise. These are not fulltime permanent positions.
