Table of Kingz Registration & Enrollment Forms

Thank you for enrolling your Young King/Queen in the Table of Kingz/Queenz. Please complete the enrollment and registration forms, including waivers of liability and media release forms. If enrolling more than one child, a separate form is required for each participant.

By submitting the forms, you agree to the following:

  1. The accuracy of the participant's information, answered by the parent/guardian or participant.

  2. Acceptance of the program's terms, code of conduct, and attendance policies.

  3. Submission of required uploads on behalf of the participant.

  4. Acceptance of payment terms for enrollment.

  5. Agreement to waivers of liability and media release forms.

  6. Acknowledgment of your email and legal name (and the participant’s) as a digital signature.

Table Of Kingz/Queenz: Meeting Frequency
Table Of Kingz [TOK], Table Of Queenz [TOQ] is a 10 week program, that meets 3 days a week--twice a week after school, Monday-Friday, and once during the weekend on a designated Saturday or Sunday. A calendar of the cohorts designated cycle days will be provided to you. Please note that the calendar dates are subject to change. Advanced notice of such changes will be provided to the parent/legal guardian via email, and/or text or phone call as secondary means of communication when necessary.

Additionally, a 1 year commitment to "pay-it-forward" is required of all participants after the Crowning Ceremony. This commitment consists of your Young King/Queen serving as a peer mentor to other Young Kingz/Queenz within future cohorts and participating in various community service activities with TOK. Information on activities and workshops will be provided with advanced notice via email, text or by phone call.
