CSBA Fall 2025 Trial Class Day Sign-Up
Welcome new students!  Try out a class before the Fall Semester begins!  Fill out this form to reserve your space for our TRIAL CLASS DAY!  

All classes are held on SATURDAY, SEPTEMBER 13th at 440 Lafayette.  

Please note that trial classes (with the exception of Pre-Ballet) are abridged by 15 minutes from our regular class lengths in those levels.  

Trial students may wear ballet uniforms of their own, or borrow from CSBA. For reference, CSBA uniform requirements can be found here:  http://www.chrystiestreetballet.com/uniform-policies

Once your child's reservation for the trial class is confirmed, an invoice in the amount of $25 will be sent to the email address you provide on this form.  Placement will be held/confirmed upon receipt of payment.  Please note space is limited!

Please note this is NOT a registration form for Fall classes.  If you wish to enroll for Fall classes, please go to THIS form instead.
