This is the welfare concerns and incidents report form. A copy of this form will be sent to only those that you specify. 

Please note that if that committee member feels that it is appropriate to escalate the concern, they may do so to the Junior or Senior presidents.

Please also note that the Junior President has access to all reports, however will not access them unless the report is escalated by another committee member.
