This is the form used for those interested in becoming an FTO Advanced Training Instructor. The letter of interest should include your desire for the position, qualities that would make you a good fit, and what makes you unique from other candidates. Below you will find the other requirements.

- Must be an FTO.
- Must have been an FTO for at least 30 days.
- Must have completed at least 5 ride-along phases.
- Must have no 10-90s or 10-93's in the past 30 days.
- Must demonstrate the ability to complete additional responsibilities.
