Congratulations on receiving a Senior Scholarship from the Melrose Bandaiders. Please use this form to request your scholarship payment. It can be requested any time within 4 years of your high school graduation.

With this request, a check will be made out to the educational institution listed and will be mailed to YOU at the address shared in this form. I understand that this scholarship money is to be used for educational expenses at my chosen institution.
