At MadSkillz Lab, our goal is to deliver high-energy, competitive, and purposeful training sessions that build upon one another week after week. Every athlete is expected to be fully committed to the development process—not only by attending weekly sessions, but also by taking ownership of their improvement through individual training outside of practice.

We believe player development should be both challenging and enjoyable. Our mission is to create an environment where athletes work hard, compete with confidence, build lasting memories, and develop a lifelong passion for the game. Every session is designed to help players reach their highest potential while learning to love the process of improvement.

The MadSkillz Development Model

To become a complete player, athletes must develop in every aspect of the game. Our curriculum focuses on four essential pillars that separate good players from great ones.

These qualities are developed through consistent exposure to demanding training sessions, high-level competition, and a structured long-term curriculum. Our objective is not simply to create talented players—but disciplined, confident, intelligent athletes who are prepared to succeed at the next level.

Player Waiver, Release of Liability & Indemnification Agreement

By participating in any MadSkillz Lab program, training session, camp, clinic, league, tournament, or event, I acknowledge and agree to the following:

Player Acknowledgment

I certify that:

  • I am voluntarily choosing to participate in MadSkillz Lab activities.

  • I am in good physical health and capable of safely participating in athletic activities.

  • I understand that participation in soccer, futsal, strength and conditioning, and related activities involves inherent risks that cannot be eliminated.

These risks include, but are not limited to:

  • Falls and collisions

  • Contact with other participants

  • Equipment failure or misuse

  • Playing surface conditions

  • Weather-related conditions

  • Sprains, fractures, concussions, serious injury, illness, permanent disability, or death

I voluntarily assume all known and unknown risks associated with participation, including while:

  • Participating in training sessions, camps, clinics, games, tournaments, or special events;

  • Serving in any non-playing capacity during MadSkillz Lab activities; and

  • Being present at any training or event facility before, during, or after participation.

Release of Liability

To the fullest extent permitted by law, I release and forever discharge MadSkillz Lab, its owners, directors, coaches, instructors, employees, contractors, volunteers, affiliates, facility partners, sponsors, and representatives from any and all claims, demands, liabilities, damages, costs, expenses, or causes of action arising from or related to my participation, including claims resulting from ordinary negligence.

This release applies to any injury, illness, property damage, loss, disability, or death that may occur during participation in MadSkillz Lab activities.

Parent/Guardian Agreement (For Participants Under 18)

As the parent or legal guardian of the participant, I certify that:

  • My child is physically able to participate in MadSkillz Lab activities.

  • I understand and accept the inherent risks associated with participation.

  • I authorize my child to participate in all approved MadSkillz Lab activities.

On behalf of myself and my child, I voluntarily release, waive, and discharge MadSkillz Lab, its owners, directors, coaches, employees, contractors, volunteers, affiliates, facility partners, sponsors, and representatives from any claims arising from participation, including claims based on ordinary negligence, to the fullest extent permitted by law.

I further agree to indemnify and hold harmless MadSkillz Lab from any claims brought on behalf of my child that arise from participation in the program.

Acknowledgment

By selecting "I Agree" or signing this agreement electronically or in writing, I acknowledge that:

  • I have carefully read this Waiver, Release of Liability, and Indemnification Agreement.

  • I fully understand its contents.

  • I understand that I am giving up certain legal rights.

  • I sign this agreement voluntarily and intend for it to be legally binding on myself, my heirs, executors, administrators, and assigns.
