WE NOW HAVE 5 LOCATIONS

Our goal is to run high energy, competitive, challenging and dynamic training sessions that build off each other. Every athlete must be fully committed to the training program and is expected to not only show up for training once a week but also train on their own.

It's all about having fun while working hard and creating memories. Another goal of ours is to ignite a passion for the sport in each and every athlete that we train. We teach skills to every athlete in hopes that they will play a lifetime.

For a Player to be considered a top player they must excel in all areas below. 

These elements are developed by exposure to highly challenging, weekly training sessions and frequent highly competitive matches. This will insure development of the following vital components of the highly recruitable player:

- TECHNICAL ABILITY
Dribbling, ball control, skill moves, passing and receiving, 1v1 attacking and defending.

- TACTICAL AWARENESS
Small sided games, possession based activities 

- PHYSICAL
Agility, cone work, sprints, running, playing the game, fitness etc.

- MENTAL  
self-determined motivation, environmental and surrounding factors, along with other personal forces such as persistence and optimistic thinking.

Player Waiver, Release of Liability and Indemnification Agreement




I, the undersigned player, acknowledge, agree and understand that:

1. Voluntarily and of my own free will, elect to participate in the MadSkillz Futsal PROGRAM. Furthermore, I agree that I am in good health and proper physical condition to participate.

2. I understand that there are certain risks and hazards involved in participating in this activity that may result in injury, illness or death to me or other players including, but not limited to those hazards associated with, playing conditions, equipment, and other participants. Further, I, the undersigned player, agree that in consideration for the right to play futsal and in consideration for permission to play:

1. I voluntarily elect to accept and assume all risks of injury or illness incurred or suffered by me 

(a) while practicing or playing as a member of the team so designated, 

(b) while serving in a non-playing capacity as a team member during practice or play by other teams or by other players on my team, and 

(c) while on or upon the premises during league play or practice.

2. I hereby release, discharge and agree not to sue MadSkillz FC program or their owners, officers, agents, servants, associations, employees, or any person or entity connected with futsal or any claim, damages, costs or cause of action which I have or may in the future have as a result of injuries or damages sustained or incurred by me from whatever cause including but not limited to the negligence, breach of contract or wrongful conduct of the parties hereby released.

Release of Liability and Indemnification Agreement

I, the undersigned parent or guardian of the above named minor, acknowledge, agree and understand that:

1. The above named minor is in good health and proper physical condition to participate in futsal.

2. There are certain risks and hazards involved in the above named minor participating in futsal that may result

in injury or death to the minor or other players including, but not limited to those hazards associated with, playing conditions, equipment, and other participants.

3. I hereby release, discharge and agree not to sue MadSkillz or their owners, officers, agents,

servants, associations, employees, or any person or entity connected with MadSkillz for any claim, damages, costs or cause of action which I have or may in the future have as a result of injuries or damages sustained or incurred by the above named minor from whatever cause including but not limited to the negligence, breach of contract or wrongful conduct of the parties hereby released. I ACKNOWLEDGE THAT I HAVE READ AND THAT I UNDERSTAND EACH AND EVERY ONE OF THE PROVISIONS IN THIS WAIVER, RELEASE OF LIABILITY AND INDEMNIFICATION AGREEMENT AND AGREE TO ABIDE BY THEM.
