Please use the form below to sign in to our event, so we have a record of your attendance.

This form allows for electronic sign-in to Lean-related events sponsored by the Office of Continuous Improvement. These events include Lean Workshops, Lean training, guest speakers, visitors, and the like, which are hosted by the Office of Continuous Improvement (OCI). Choose "OCI Organized Event." OCI will update the spreadsheet with the name of the event and you don't need to enter the name of the event.

In addition, you may use this form to record other Lean related activities for your Lean portfolio. Choose "Other - Describe in optional note area (next)," then describe the activity in the "Optional Personal Note or Comment" field.

Finally, this form is used by Lean training homework coaches to record their contacts with their trainee. Choose "Homework Coach" and complete the additional fields.

All facilitators have view access to the spreadsheet. You may view and sort at any time.

Periodic independent validation of attendance will occur. If you should accidentally enter incorrect attendance information, please notify OCI at improvement@mtu.edu so we can fix it.
