This form is to be used to file an official misconduct complaint with the Los Santos Police Department Internal Affairs Division (PDIA). All submitted information is considered confidential and anyone who files a complaint is expected to keep all details pertaining to their case to themselves and the LSPD Internal Affairs Division.

If the complaint you are filing is on two separate people, FILL OUT THE COMPLAINT FORM TWICE, one for each person. You may copy and paste the details from the first response into the second, but for organizational reasons, LSPD IA WILL REQUIRE TWO SEPARATE RESPONSES.

This form is only used for complaints of misconduct involving Los Santos Police Department employees. The LSPD Internal Affairs Division does not investigate employees of other State departments/agencies, such as the San Andreas Highway Patrol or Blaine County Sheriff's Office.

The LSPD Internal Affairs Division does investigate complaints against individuals for any out of state related complaints. (All OOC, FailRP, ForceRP, or other R&R related complaints should be directed to MWIA). Please fill out the MidwestRP Internal Affairs complaint form.
