HQHT Connections is a unique social learning/social encounter group that focuses on providing opportunities for developing self advocacy, perspective taking, self-awareness and increased self determination and confidence. We look forward to working with your child to provide neurodiversity affirming social development learning and growth opportunities. Please use this form to provide us with information about your child so that we may create the best social opportunities and fit. After we receive this intake form, a member of our team will be in touch to schedule a virtual or in person intake meet and greet with you and your child prior to group placement so we can determine the best group fit.
