"Thank you for trusting upDog Pet Services, LLC. Please complete this waiver before your dog's first visit to ensure we provide the best and safest experience. By submitting this form, you agree to our policies and guidelines outlined below.
Is your dog up to date on Rabies, Bordetella and Distemper/Parvo vaccinations? (Please submit copy of records to updogpets@yahoo.com) *
Is your dog on a flea and/or tick and/or heartworm preventative? *
In the event of a medical emergency, I authorize upDog Pet Services, LLC to seek veterinary care for my pet at their discretion. I understand I am responsible for any veterinary expenses incurred during my dog's stay. Do you agree to this policy? *
I agree to provide clear instructions for my dog's feeding schedule and/or medication needs. I understand that while staff will administer medications as directed, upDog Pet Services, LLC is not liable for any adverse reactions. Do you agree to this policy? *
I confirm that my dog has no history of aggression towards other dogs or humans. If my dog displays any unsafe behavior during their stay, I understand they may be removed from group play and charged accordingly OR require immediate pick up. Do you agree that your dog may be re-assessed after extended absences or in cases of behavioral concerns? *
I understand that if my dog participates in group play, socialization and play involve inherent risks, including but not limited to minor injuries (e.g. nicks, scratches) or the rare risk of more serious incidents. I accept these risks and release upDog Pet Services, LLC from liability or injuries resulting from normal play interactions with other dogs. Do you understand the risks associated? *
I understand that upDog Pet Services, LLC is not responsible for any lost or damaged personal items (e.g. leashes, bedding, collars, toys) brought for my dog's stay. Do you agree to this policy? *
Do you agree to text us when you're on your way and upon arrival, and to wait in your vehicle until directed? *
Do you give permission for upDog Pet Services, LLC to use photos or videos of your pet for social media and promotional purposes? *
Do you agree to provide at least 24 hours' notice for cancellations and understand that late pickups may incur a fee? *
For stays longer than 5 days, do you understand and agree to provide a non-refundable 20% deposit at the time of booking to secure your dogs spot? *
I understand that payment is due at the time of service, and deposits as mentioned above are non-refundable. In addition, no refunds will be issued for early pickups or cancellations made without required notice. Do you agree? *
Do you release upDog Pet Services, LLC from any liability related to injury, illness, or damages while your dog is in our care? *
In the event of inclement weather or natural disaster (e.g. Hurricanes) I understand that early pickup may be required. I agree to provide an emergency contact if I can not be reached so that my pet may be evacuated promptly.  *
I understand that if I fail to pick up my dog on the agreed upon date and make no contact within 7 days, upDog Pet Services, LLC reserves the right to consider my dog abandoned and take appropriate action in accordance with local laws. I will be responsible for any fees incurred during this time. Do you agree? *
