As we work to move to a new teaching and learning model for this spring, we would like to identify any of our students who may not have access to devices and internet access that might be needed to communicate back and forth with teachers.

Please complete this short survey to let us know if we need to help you get a school-issued device and if we need to help find ways to connect to the internet.  If your child has a device and internet access, you do not need to complete this form.
