If you would like Mr. Raak to write you a Letter of Recommendation, please have a conversation with him in person first to confirm that it can be done in time.

Once you have talked with Mr. Raak, please fill out this questionnaire if you would like him to write you a Letter of Recommendation for a Summer Program, Internship, or Volunteer Opportunity that you would like to pursue.

The more information you provide the better your Letter of Recommendation will be.

If you would like more than one Letter of Recommendation, please fill out a new form for each letter.

Mr. Raak cannot deliver letters of recommendation directly to students.

Program uses website link. (Student will submit the email address of the recommender, and the recommender will be sent an email link to upload the rec)

Recommendation is mailed. (Student will print out a recommendation form or cover sheet and submit it to the recommender with a stamped and addressed envelope)

Recommendation MUST be sent in one complete application package. (Have the teacher submit the letter to your counselor. Make an appointment with your counselor to organize the application package)
