Food Preparation and Service Technology (FPST)

1. Please fill out the form carefully and truthfully. Make sure that the information you will provide in every category is accurate before you submit.

2. ENTRANCE EXAMINATION is not required in the enrollment process for this school year only. However, the student will undergo the Diagnostic Examination by the subject teachers when classes start to asses the level of academic competency.

          2. Good Moral Certificate (Original and Photocopy)
          3. Birth Certificate (PSA-Original and Photocopy)

C. Transferee Student
