EMAT Awards Nomination Form 2019
Please fill out the Nomination Form in its entirety. Incomplete forms will not be considered. All nominations must be submitted on or before February 1, 2019.

Self-nominations will NOT be accepted. Nominations must be submitted by an EMAT member although nominations can be for non-EMAT members (unless specified in the award criteria). There is no limit to the number of nominations an EMAT member may submit. There is no entry fee to submit a nomination.

Nominations should reflect activities and programs which occurred during the past calendar year (January 2018 – December 2018) with the exception of the Lifetime Achievement Award. For projects and programs that span longer than previous 12 months, the vast majority of the project/program activities must have occurred during the previous calendar year.

Please email all documentation that helps justify the award recommendation to ematawards@gmail.com. This documentation should be available for posting on EMAT's website for member's use and for display at the annual conference.

EMAT Awards Committee Judging Process
During the Judging Process, the EMAT Awards Committee will evaluate nominations based on the following information:

• Award selections will be based on the information presented in the award nomination

• Nominations need to be clear, descriptive and provide specific examples that clearly demonstrate that the person/project/program is worthy of the selected award

• Eligibility requirements:
o Is the nominator an EMAT Member?
o Does the nomination reflect activities that occurred within the past calendar year (2018)?
o Is the nomination a self-nomination?
o Is the nomination submitted for the correct award (based on the award criteria)? (Note: It is at the discretion of the Awards Committee to move nominations from an award category to another during the Judging Process.)
