Guidance Notes
Please ensure this form is completed at least 4 weeks prior to requesting leave from the Academy. It can be used for requesting any absence such as holiday request, request for celebrations such as EID and any other absences during term time. Completing this form does not mean that your request has been approved.

The Education (Pupil Registration) Regulations 2013 state that
Headteachers may not grant leave of absence during term time unless there are exceptional
circumstances (see below).

You will receive written confirmation of the decision made by
the Principal.

If your leave of absence is approved and you fail to ensure your
child/children returns to the Academy by the agreed date or your child is
absent when your leave of absence is not approved, the absence will be marked
as unauthorised on the register.

For unauthorised absence accrued you may be liable for a penalty
notice (fine) issued by the Local Authority. The fine is £120 per parent
per child, reduced to £60 per parent per child if paid within 21 days. If one
or both parents/carers fail to pay the penalty notice in full, then you may be
prosecuted. The Academy also reserves the right to remove your child from the
Academy roll if your child does not return on the date given if over 20 school
days.
Exceptional circumstances: In consideration whether any exceptional circumstance apply, the Principal will consider if the reasons are rare, significant, unavoidable and short. The Principal will also take into consideration the factors listed below:
whether the event for leave of absence is requested can reasonably take place during school holidays.
levels of attendance and unauthorised absence over the last 12 months.
any leave of absence previously.
whether the leave is requested during the exam period or during a controlled exam period.
age and year group of the pupil.
