UIC Music Audition Request Form
Please complete this form to request an audition. Once your form is received we will assign you an audition time with our Faculty. You will receive an email confirmation with your audition details shortly thereafter.

Please do not request an audition unless you have already:
1. applied to the university via the Common Application
2. submitted both your transcript and standardized test scores to UIC

Also please note that this form is not an application to the University.
