The submission of this form will place your child on a waitlist so that Monument Academy Homeschool Partnership knows how to reach out to you when the enrollment process begins for your child. One submission is required per student. This information is considered confidential and will not be shared outside Monument Academy administrators.  

** Please note that at this time, only grades K-5 will be offered for the 2024-25 school year. Additionally, part-time high school enrollment may be offered on a case-by-case basis for the 2024-25 school year. **
