Please be advised that completing psychological testing, preliminary health questionnaires, and/or new patient intake screening with staff does not establish a clinician-patient relationship with this practice.  First your clinician will review your psychological testing, intake screening, and health history.  Then your clinician will conduct an initial evaluation virtual medical appointment to determine if you are a suitable candidate and if the practice will accept you as a patient.
