Locker Request
Please fill this out if your student wishes to be assigned a locker for the upcoming school year.  A $2 locker fee will be charged to your school fee's account.  Lockers will only be assigned on a request basis; no student will be assigned a locker unless they complete this request form.  
The student is responsible for keeping their lockers clean and in good condition. If damage is done to the lock or locker, a fine may be assigned for the replacement of each item.

Instructions for Requesting a Locker:
  1. Fill Out the Form: Please complete all required fields in the form accurately. This includes your student's name, student ID number, and any specific requests or needs regarding lockers.
  2. Monitor Your Student's Skyward: After submitting your form, keep an eye on your student's Skyward for updates. The assigned locker will show up in Skyward once it is assigned.

Locker Usage Guidelines:

  • Respect School Property: Lockers are a privilege and must be treated with care.
  • No Stickers Allowed: To maintain the condition of the lockers, do not place any stickers on or inside them.
  • Keep It Clean: Ensure your locker remains clean and organized. This helps in maintaining a tidy and efficient school environment.
