In order to ensure each child gets adequate coaching attention, FREE Trial Classes are scheduled as availability allows.  After submitting your request, you will be contacted by email with the date/time of any available classes to choose from.  Your reservation for class is not complete until you have received email confirmation of your assigned slot and additional class instructions.

WAIVER/INDEMNIFICATION --

As a parent or legal guardian of the child named in this registration, I hereby represent that the child has been examined by a pediatrician and is physically able to participate in this or any other athletic or social event taking place within the Legends Indoor Soccer Facility. I further represent that neither I or my child has been in contact with anyone who has been exposed to or who currently has the Corona Virus within the past 14 days. I understand there are inherent risks in participating in this athletic program and hereby accept responsibility while in agreement to pay any and all costs of medical treatment resulting from injury suffered by my child as a result of his/her participation in this event or any other event held at the St. Louis Legends Facility.

I further agree to indemnify and hold harmless HappyFeet St. Louis LLC; its agents, coaches, servants, employees, and or representatives from any and all liability, damage, or expense arising out of my child's participation in HappyFeet St. Louis LLC events or events conducted by other entities while on or in the St. Louis Legends Indoor Soccer Facility. I hereby give permission for care to be administered by a qualified St. Louis HappyFeet LLC staff member, EMT, physician/staff of a hospital, or any other qualified individual to provide medical treatment deemed necessary for my child.
