This form is completed annually by each proprietor's representative on a Board in the Catholic Diocese of Dunedin. It is confidential to the Bishop, the Vicar General and the Education Office. The results may be aggregated and shared with the Board a feedback tool to improve process and safe guard the special character of the school or college.
