Neal Shusterman Visit Excusal Application
Fill out this form to be excused from class to attend Neal Shusterman's live presentation on Monday, 11/28 (A day). You will miss the second half of first period and advisory. You will need to leave first hour at 8:20 AM to get to the PAC on time for Neal Shusterman's 8:30 presentation. 

You must sign up by Monday, 11/21 to attend Mr. Shusterman's presentation. 

NOTE: If you have English during first period on Monday, 11/28, you will attend the assembly with your English class. You still need to fill out this form!

FYI: Neal Shusterman will sign 150 copies of his new book, Gleanings, for Alta High students. 125 signed copies will be available for purchase in the front office for $14. The books will be sold on a first come, first serve basis. It's recommend that you preorder the books now by paying for your copy in the main office. You will pick up your book in the library AFTER Mr. Shusterman's presentation on 11/28 by showing your receipt to the library staff.

There will be a raffle in the library for the remaining 25  signed copies of Gleanings. These books will have personalized dedications from Mr. Shusterman to the raffle winners. If you win one of the 25 copies, you will not need to pay for the book. You can fill out a raffle ticket in the library starting November 1 (there's also a link in the "Thank you for responding page" that will take you directly to the Google Form raffle!).

1. You promise to behave respectfully and listen attentively to Neal Shusterman AND to the students who are chosen by Mr. Shusterman to ask questions in the audience. 
2. You promise on your life and on all your favorite books that you WILL NOT record Neal Shusterman on ANY electronic device. 
3. You agree that any questions that you may ask in the assembly will be school appropriate, respectful, and pertinent to the presentation itself. 
4. A breech of this agreement may result in disciplinary action.
