This form should be completed for each delegate, advisor, or assistant chair requiring a Letter of Invitation or Itinerary in Japan form in support of their visa applications.

If you are not sure whether you require these documents, visit https://www.mofa.go.jp/j_info/visit/visa/index.html, and under the “Short-Term Stay” section of “Visa Application Procedures”, simply click on the colored button that corresponds to your country.Then, download the PDF which outlines application instructions for single-entry visas from your country, and see if these forms are required under the "Tourism" column.

Please note that paper-based visa applications will take up to 6-8 weeks to process, so we urge all delegates requiring visas to submit their application as soon as possible. If you are not sure what type of visa is required for your country, . Once we receive your request, we will issue you the Letter of Invitation and Itinerary in Japan forms.
