Clayton Math Pentathlon Pre-registration 2016 - 2017


The Mathematics Pentathlon (MP) is a program of interactive problem-solving games, supportive curricular and instructional activities, and assessment tools for students in grades K-7. By using specially designed games and activities that differentiate instruction, ALL students of varying abilities and learning styles are motivated to enjoy math and spend more time learning this subject matter. The Clayton MP teams meet Tuesdays after school from 2:57 - 3:57 pm on the Clayton campus.
After the teams are assigned, we will let you know if your child is part of the team and where they will practice. Registration does not guarantee a spot on the MP team. Students that participated in the previous year tournament will have priority then the team will be open to additional students. You will be contacted by September 24th, 2016.

In order to be a member of the club, the following requirements MUST be met:
- Student must attend as many practices as possible.
- Participants must abide by the contracts outlined below.

In addition, a qualified adult volunteer must:
1. Commit to volunteering a MINIMUM of 4 practices per session (Session I: October - December, Session II: February - April), per child enrolled.
2. MUST learn and be able to monitor 3 of the 5 games. (They will monitor these games at the tournament.)
3. Participant and a qualified adult volunteer MUST attend the tournament in the spring.

* Please do not reserve a spot on the team if you cannot commit to tournament. Last year we had several children turned away due to space limitations.

Spring tournament dates are:
Division I (K-1): April 1st, 2017
Division II (2-3): April 8th, 2017
Division III (4-5): April 29th, 2017

The registration fee is $150 per child. All fees are NON-REFUNDABLE.
The fees include one snack each week, tournament participation, one child t-shirt, one adult t-shirt and end-of-the-year party.

Upon acceptance to the team, you will receive an email with verification and information on submitting payment.

    Student & Parent Commitment Agreement 2016 - 2017

    The Clayton Math Pentathlon Club is a program run by parent volunteers, for students who want to learn math skills, strategies, and academic sportsmanship. The goal of Math Pentathlon is to increase each student’s math confidence and problem-solving skills. In order to ensure that parents and students understand the responsibility and commitment needed by each student, please take time to read over and sign this contract with your child.

    Student Contract

    Please read (or have your child read) each of the following statements:  I realize that the best way to learn is to work cooperatively as a group, to share my ideas with confidence and to accept others ideas with an open mind and a positive attitude.  I agree that my behavior at all team meetings and events will be constructive and in agreement with the Clayton Math Pentathlon Club Student Code of Conduct.  I agree that I will show good sportsmanship and fairness in any games that I play.  I agree that each Math Pentathlon team practice is valuable. I will attend each meeting as best as I can.  I agree to follow the directions of the coaching team and to not be disruptive of game play, group discussions, or activities.  I agree to respect all adults within the coaching team and all of my fellow teammates at all times.

    Parent Contract

    Please read each of the following statements:  I accept that membership in the Math Pentathlon program is a privilege that can be revoked if a student does not fulfill my obligations in this student contract as well as behave in agreement with the Clayton Math Pentathlon Club Student Code of Conduct (see email.)  I realize that the Math Pentathlon volunteer parents will be contributing a significant amount of time and effort to provide a rewarding experience for my student. I will try to be as cooperative and helpful as possible.  I agree to discuss all items listed above with my child and reinforce these items weekly.  I respect and appreciate the time of the volunteers and will pick-up my child promptly at the end of practice.  I agree to meet my required number of volunteer hours each session.  I agree to learn the 3 games that are assigned to me as a monitor.  I agree to commit to having my child at tournament with a qualified adult that will monitor my assigned game(s).
