Laptop/Device(s) Use Agreement Form

Para Español Laptop computers and devices are valuable, fragile and desirable commodities. It is important that special

precautions are taken to protect and guard them. The following safeguards are to be implemented and observed:

  • The Laptop/Device(s) listed below is issued to you and should be used by Extera students only, for educational purposes

  • You are responsible for the Laptop/Device(s).  If it is lost, stolen or damaged from misuse, you will be liable for replacing or repairing the Laptop/Device(s)(s)

~I hereby accept responsibility for the loss of, theft, or any damage to the Laptop/Device(s) listed below. I further agree to hold Extera Public Schools, its governing board and its officers harmless of any and all liability arising from any injury or loss resulting from the condition or use of this laptop. I agree to return the technology to Extera Public Schools when requested. If I damage or lose the device, the school site will conduct an investigation to determine if the damage was accidental, willful, or negligent and such damage could result in a) loss of access to the device b) financial responsibility c) in school suspension or work in lieu of payment d) TechSchool.

Please note we are entered into a warranty and repair agreement.  DO NOT ATTEMPT TO REPAIR THE DEVICE ON YOUR OWN.  Doing so will void the warranty and you will be responsible for the full cost of the device.
