Student's Code of Conduct - To be read and agreed to by Student
The Student agrees to attend all practices and to follow the direction of coaches. In the event of illness or family circumstances where the student is unable to practise/attend games, coaches must be informed ASAP.

The Student agrees to play for their team for the full season.

The Student agrees to wear the full team uniform at all matches.

The Student is to be on their best behaviour at practices and games.

The Student understands that failing to follow the above agreements, they will be excluded from practices and matches as determined by the school principal and coaches.

Parent/Caregiver's Code Of Conduct
I have read the Student's Code of Conduct and I will ensure that the student will abide by the by the Code as set out.

As a parent/caregiver and spectator, I have read and agreed to the the Spectator's Code, as presented on the School Sports enquiry page of the Woodend Primary School website.

I will make all efforts to ensure the student attends all training sessions. In the event of illness or family circumstances where the student is unable to practice/attend games, coaches must be informed ASAP.

Regarding sports that involve fees, I agree to pay the sports fees associated with the following sports by the folliowing timelines;
- Cricket, Basketball - payment of fees by Week 6, Term 1.
- Netball, Soccer, Football - payment of fees by Week 4, Term 1.
- Volleyball - payment of fees, 1st season Week 11, Term 1 - 2nd season Week 10, Term 3
Failure to pay fees within the timeline will result in them not receiving their trophy or medal. The school, not your co-ordinator/coach will manage this process.

I agree that should the student's uniform be damaged or lost, I will pay for any repairs or replacement.

I have filled out the the above Medical Information correctly and understand that if the student requires medical treatment, I am liable for the cost of such treatment.
