OCSD Employee Recognition Form
Use this form to nominate an Ottumwa Community School District staff member (teacher, administrator, and non-teaching staff) who has demonstrated going above and beyond for students, staff, and parents on a continuous basis (as an example: a staff member tutoring students outside of their work day). Submitted recommendations will be reviewed and five will be recommended to the Superintendent to be honored at the regular monthly board meeting. Employees not selected will be considered in the following months.
