Photography/Video: The Eagles Mere Athletic Association (EMAA) periodically updates our written and electronic publications ("EMAA publications") to include photographs, digital images, and videos of EMAA program activities and events. Your image and likeness may be captured in these photographs, digital images, or videos ("EMAA photographic materials"). By checking the box, you: Acknowledge that the EMAA has complete ownership of EMAA photographic materials; authorize and permit the EMAA to use your name and image in EMAA publications, in the EMAA's sole discretion; and fully release the EMAA and its employees, volunteers, directors, and representatives from any and all claims by or on behalf of you arising out of or in connection with such use of your name and image in EMAA publications.

Although the EMAA strives to provide safe activities for participants, participation in EMAA activities necessarily involves the risk of injury. By checking the box below you: acknowledge that you understand that your participation in EMAA activities involves the risk of injury, including serious injury or death; voluntarily choose to participate in EMAA activities despite this risk; represent to the EMAA that you are physically and emotionally capable of participating in EMAA activities and have no physical or emotional disability or restrictions that would restrict full participation in EMAA activities; release the EMAA, and its employees, volunteers, directors, and representatives, from all claims by or on your behalf arising out of or in connection with any EMAA activity, including but not limited to claims for physical or emotional injury or death; and agree to defend and indemnify the EMAA, and its employees, volunteers, directors, and representatives from all claims by anyone, including claims by or on your behalf, arising out of or in connection with your participation in any EMAA activity.
