Welcome to the Panel Request form! Please fill this out with as much detail as possible. After you submit, you will receive an automated confirmation email & we will be in contact with you to finalize the panel.

-Our target is to serve PSU students and that is reflected through this form.
-We require minimum two weeks' notice to schedule panels.
-Minimum audience size is 10 students. If your class is smaller than 10 students, consider partnering with another small class to co-host Queeries!
-We expect all participants to adhere to our group agreements and ask for your cooperation in making panels fun, safe, and productive. We reserve the right to cancel any panel that conflicts with these goals.
