2020 Jr Tennis Camp COVID-19 and Policies
This form is mandatory to complete for all campers participating in the 2020 Junior Tennis Camp at Upper Dublin Sports Center, powered by Tennis Match Academy. Please complete this form in its entirety prior to the start of camp for your child. Please do not hesitate to reach out with any questions or concerns.

2020 Junior Tennis Camp Policies
It is understood that any discounts received are for commitment to attending the actual days and times purchased.

Prepaid full weeks cannot be broken up into individual days and must be used during the actual weeks registered and paid for. Under no circumstances will days be changed or refunded.

To complete registration, payment must be received in advance.

If your child falls ill or injured, a credit will be placed on their account for future use. There will be no REFUNDS or MAKE-UPS.

If someone in the camper’s household or camp group is diagnosed with, or exposed to, COVID-19, they will be
required to quarantine at home for 14 days. If this happens, camp fees may be refunded on a prorated basis.

Both indoor and outdoor courts will be used, with an emphasis on outdoor play.

I certify that my child is physically fit and have not been advised to not participate by a qualified medical professional. On my own behalf and as parent and/or guardian, I acknowledge and agree that there is the possibility of physical injury or loss associated with my Child's participation in the Upper Dublin Sports Center. I hereby release and discharge Upper Dublin Sports Center, employees, and associated personnel including the owners of the facility against any and all claims, liabilities, and/or damages as a result of my Child's participation at Upper Dublin Sports Center, including but not limited to any claim that Upper Dublin Sports Center was negligent. I further agree to defend and indemnify Upper Dublin Sports Center, junior tennis camp, its employees, and associated personnel if any claim is made against them by or on behalf of my Child. I authorize Upper Dublin Sports Center Tennis Camp program to obtain, store, and/or use (without payment) any photographs, slides, and/or videotapes of my Child for public relations, marketing and advertising, and/or internal training purposes, including any social media efforts. I understand my Child will not be permitted to participate in the Program without me signing this Agreement.
PARENT/GUARDIAN COVID AGREEMENT:
To protect our children and staff, I agree to keep my child at home if he/she has:
- Fever (a temperature of 100.4 or more)
- Cough
- Sore Throat
- Chills
- Muscle Pain
- Headache
- New loss of taste or smell

If my child has any of these signs of COVID -19, I will not send him/her back to tennis camp until:
- My child tested negative for COVID and is otherwise well enough to go back to camp
OR
- A healthcare provider has seen my child and documented a reason for the symptoms other than
COVID
OR
- All are true: 1) at least 10 days since the start of symptoms AND 2) fever free off anti-fever mediciations
for 3 days AND 3) symptoms are getting better

If my child is diagnosed with COVID-19, I will not send him/her back to tennis camp until the following:
- It has been at least 10 days since my child first had symptoms AND
- My child has had no fever off anti-fever medicines (ex: Tylenol, Ibuprofen) for 3 days
AND
- My child's symptoms are getting better

If someone in my household is diagnosed with COVID-19 or my child is exposed to COVID-19, I will keep him/her home for 14 days.

If someone in my household develops new cough, shortness of breath or two of the following: sore throat, chills, muscle pain, headache, new loss of taste or smell, I will get that person tested for COVID-19. If that person tested positive, I will keep my child home for 14 days.
