Complete this form to agree to terms for suitcase borrowing and to request a suitcase. You may borrow one suitcase at a time for two weeks with an additional week to accommodate for shipping time. You may also request a one week extension if there are no other immediate reservations for the suitcase. While suitcases are provided without charge, you or your institution will be responsible for the cost of return shipping. Due to a high volume of requests, please submit your request at least a month before your requested received by date to allow for repackaging and shipping.
