Mentorship Questionnaire

The reason for this questionnaire is to help facilitate introduction to individuals who have the experience or background that is compatible to your own. Please answer questions honestly and completely. Questions concerning gender, gender identification, sexual preferences, and personal history are asked specifically to allow either mentor or mentee to determine their comfort level with each other. If you feel that specific questions are not important to your mentorship relationship, please feel free not to share them.
