This is the registration page for Pink October Fun Run by Bandar Penggaram Surgical Society on 19th October 2019, Batu Pahat.

Venue: Dataran Penggaram, Batu Pahat
Distance of run: 5km
Fee: RM 55 per person (includes tshirt, drawstring bag, medal and some light refreshment)
Age requirement: 12 years old and above

1. Payment can be made via online banking to our Maybank account (Acct No: 551427127291 - Name: Kelab Surgikal Bandar Penggaram).
2. Please upload proof of payment (Screenshot or transaction details) in the form below. If you are unable to upload the file here, please send proof of payment to: bandarpenggaramsurgicalsociety@gmail.com with your name and IC in the email title.
3. Kindly fill up all the details below and make payment before 15th September 2019. We will email you a confirmation after we have received your payment - usually within 3 working days.
4. Payment is non-refundable, but we may facilitate a bib transfer (change of participant name) before 15th September 2019. Please contact us via email or whatsapp for this.
