Student Handbook *
I am aware of the digital copy of the EF Elementary Student Handbook and the EFISD Student Code of Conduct that is posted on the web site www.efisd.org and agree to comply with the policy within them.  I understand that the handbook contains information that my child and I may need during the school year and that all students will be held accountable for their behavior and will be subject to the disciplinary consequences outlined in the Student Code of Conduct.  (A paper copy is available for you to refer to in the principal's office.)  All questions regarding the handbook may be directed to the principal or assistant principal at 903-633-2465.
Artwork, Photos, etc *
I give permission for my child's work to be electronically displayed and produced by the district.  This allows the district to display accomplishments such as A-B Honor Roll, etc. on the school website, etc.
Electronic Communications System *
I give permission for my child to participate in the district's electronic communications system.  (This allows the student to use the district's computers.)  I understand that my child's computer use is not private and that the district will monitor all activity on the school's computer system.  I also agree that my child will not participate in cyber-bullying activities.  I agree that I have read the district electronic communications system policy and administrative regulations and agree to abide by their provisions.  I understand that violation of these provisions may result in suspension or revocation of system access.  
Personal Identifiable Information *
Permission for certain personal identifiable information about my child or a photograph to be electronically displayed and produced by the District.  (This allows the students to have their photos used in the yearbook, newspaper, and on the school website--ex:  extracurricular activity photos such as football, cheer, ag, etc.)

School Health Services *
We, the parent/guardian, understand that emergency medications may be provided, as part of the Emergency Protocol, by the District.  medications on the emergency protocol may include the name brand or equivalent of the following:  Epinephrine, Albuterol 90 mcg inhaler, and Oxygen.  We understand that students who have previously been diagnosed with life-threatening allergies are required to provide their own prescriptions.  We also acknowledge that it is our responsibility to notify the school office of any changes to emergency contacts and/or phone numbers.
