Interstitial Lung Disease Interdisciplinary Network (ILD-IN) Award Nomination Form 2024
We invite you to nominate your healthcare professional(s), colleague(s) and/or team member(s) for recognition of their contribution to interstitial lung disease and for going above and beyond to make a difference. You can vote for as many people and/or team(s) as you like.  

There are 3 award categories:

1. Patient (nominated) Award
Only patients can nominate and the award will recognise a person or team who has/have contributed above and beyond to the care of the patient and will inspire others to deliver high quality patient care.

2. Rising Star Award
Any member of the ILD team (this includes the wider team of healthcare professionals, administrative team, charity organisations, pharmaceutical companies) can nominate and the award will recognise a person or team who is early in their career (first 2 years) and making a significant contribution to the ILD specialty. The individual or team can work in any healthcare setting (primary, community, secondary, tertiary and voluntary and research). The nominee can be any member of the multidisciplinary team, e.g. healthcare professionals, administrative team.

3. Innovation and/or use of Digital Technology Award 
Any member of the ILD team (this includes the wider team of healthcare professionals, administrative team, charity organisations, pharmaceutical companies) can nominate and the award will recognise a person or team who is leading the way and innovating to improve the care of patients with ILD. This includes any innovation which implements the use of digital technology. The individual or team can work in any healthcare setting (primary, community, secondary, tertiary and voluntary and research). The nominee can be any member of the multidisciplinary team, e.g. healthcare professionals, administrative team.

Nomination deadline is 2nd September 2024

All nominees will be recognised for their contributions to ILD care. The ILD-IN committee will review nominations and shortlist 3 nominees to receive an award which will be presented at our inaugural award ceremony at the ILD-IN conference. The patient nominated award will go to the person with the most nominations. To avoid potential conflict of interest, you can nominate any person or team but members of the ILD-IN committee will not be eligible to win an award.
