2023 NC 4-H Hall of Fame Award Nomination Form

The Hall of Fame Award recognizes 4-H volunteers, Extension professionals, staff, employees, donors and others who have made a significant impact on the 4-H program and/or 4-H members through the contribution of time, energy, financial resources, etc. This support can be provided at the local or state level.

The deadline to submit nominations is October 1, 2023.
