Public health supply order form

Use this form to order supplies for collecting and shipping clinical or animal specimens that will be submitted to the State Lab at the Colorado Department of Public Health and Environment. Clinical tests include COVID-19, tuberculosis (TB), influenza, and sexually transmitted infections. Animal tests include rabies and West Nile virus. 

The State Lab does not provide over-the-counter rapid antigen COVID-19 tests. 

This form is only open to the following groups:

  • Local public health agencies (LPHAs) in Colorado

  • Epidemiologists

  • Health care professionals

  • State facilities (i.e., DOC, DHS)

  • Hospice facilities

  • Animal hospitals and services

  • CDPHE-approved submitters (WGS, C. diff, Bordetella pertussis)

Review the sample submission process to ensure that samples are appropriately ordered, labeled, packaged, and transported to the State Lab.
