Fundraiser Approval Request Form
Please fill out this form for any fundraiser you would like to have your group conduct. All fundraisers must be pre-approved as to avoid duplication and multiple fundraisers simultaneously. Once you have received approval, you may then begin your sales.

Fundraising activities to support children's health and school nutrition-education efforts will allow food or berverages from one-half hour after school to midnight and school fundraising days (9 for high school, zero for middle school) allowed following Illinois 305 fundraising guidelines. Schools will also encourage fundraising activities that promote physical activity.

All fundraising activities must be approved before requesting purchase orders or scheduling the fundraising activity. This pertains to all fundraisers (large or small) even if it is done every year. All monies collected MUST be submitted to the office on a DAILY basis (for your protection). Monies collected over weekends and holidays should be secured and returned to the office when it reopens. *
