In order to travel with the North Summit FFA on any of their field trips you and your parents must complete the following form. This form is good for all events conducted on behalf of the North Summit Summer Ag Program from June 1, 2023 to August 31, 2023.

While traveling with and on behalf of North Summit High School, North Summit FFA and the Summer Ag Program, I agree to abide by the following rules of conduct:

I will attend activities as planned by the Summer Ag program and be courtesy and attentive to all speakers and tour guides. *

Any violation of the above rules will have a consequences. All situations will be taken on a case by cases basis. I agree that the ruling of the advisors/administrators is final and I will respect that decision. *
