Dear Seymour ISD Parents and Guardians,

New legislation from the 89th Texas Legislative Session (Senate Bill 12) requires parents/guardians to complete a health services and counseling services consent form each year for their child. By Wednesday, September 17, 2025, please take a few minutes to fill out this forms online. The form asks for your permission for the school nurse and staff to provide routine care, like first aid, screenings, or giving medication with a doctor’s order and for your permission for your child to receive short-term counseling or social-emotional support, if needed. Without this completed form, we are limited in the care and support we can give your child at school. Unless an emergency situation, you WILL be contacted for ALL minor injuries (i.e. scraped knee) and any time your child feels ill.
