Learning Center TEST Scheduling Form
All testing at the Learning Center is by APPOINTMENT ONLY and requires at LEAST 3 BUSINESS DAYS ADVANCE NOTICE. Only students who have letters of accommodations for documented disabilities are eligible to take tests at the Learning Center. Please complete all sections of this form.
*At this time due to COVID-19 precautions, ONLY STUDENTS WHO ARE REQUIRED TO TEST IN CLASS are permitted to schedule testing appointments at the Learning Center without special permission from the Office Manager or Director. Please email the Office Manager or Director for special situations.
PLEASE NOTE THAT THE LEARNING CENTER DOES NOT PROVIDE ANY PLACEMENT OR ENTRANCE TEST SERVICES (English/Math Placement or HESI Entrance Exams). We NO LONGER OFFER CLEP TESTING. Please visit the CLEP website to locate a testing center.

"I acknowledge that I have read the Learning Center Testing Policy Provided in the Welcome Email along with my letter of accommodations. I agree to abide by all guidelines including the Honor Code, Advance Notice and the Late Arrival Policy. If I arrive more than 15 minutes late for my testing appointment, my professor will be notified and my appointment may be forfeited or rescheduled at the discretion of my professor. Learning Center test proctors report all evidence of academic dishonesty to the professor. I agree to provide at least 3 business days advance notice when scheduling testing appointments."
