Personal Data Rights Request Form - PSafe

General Guidelines:

We provide this form to facilitate the exercise of your rights as outlined in the General Data Protection Regulation (GDPR).

Please note that the use of this form is optional, but it proves to be an efficient and secure way to process your request.

In case the civil identity document is not attached to this form, PSafe may request additional information from the data subject to fulfill the request and prevent any potential information security incidents that may adversely affect the requester.

Confirmation of processing existence (Allows you to know if Psafe has processed any of your personal data, even if it is uncertain whether you have been served or had any prior relationship with PSafe. (Articles 13 and 14, GDPR)

Data access (Enables you to inquire about the processing of personal data conducted by PSafe within the preceding 12 months. This option should only be selected if you have been served or had any prior relationship with PSafe. (Article 15, GDPR).

Deletion of personal data processed with the consent of the data subject (Enables the erasure of personal data processed with the consent of the data subject upon its withdrawal. (Article 17, GDPR)

Limitation of processing when the circumstances outlined in Article 18 of the GDPR are applicable.

Receiving your data in a structured format or having it transmitted directly from one controller to another, whenever technically feasible (Article 20, GDPR).

Identity Verification of Personal Data Subject

In order to ensure the rights and security of personal data subjects, after submitting the request, you will receive an email requesting photo identification documents to verify that the requester is the actual Personal Data Subject.

This proof can be a copy of an official identification document or another acceptable method that confirms the identity of the requester.

This measure is taken to ensure confidentiality and prevent the sharing of information with unauthorized third parties.

If the identity document is not sent via email, PSafe may contact you to request additional information in order to fulfill the aforementioned request.

In the event that you do not provide the requested documents, proving that you are who you claim to be, PSafe will be unable to fulfill your request.

If you are making the request on behalf of a third party (Proxy / Legal Representative)

If this Personal Data Subject rights request is made through a Proxy or Legal Representative, the requester must respond to the received email by sending the identification document of the Data Subject in question and also documentation proving the powers of representation as a Proxy/Legal Representative.

For legal purposes, documents suitable to prove this representation include a signed power of attorney or documentation proving the relationship or power of representation.

If the requested documents are not submitted, we will be unable to respond to your request.
