Nomination Form for PUSD School Board Member
By providing a nomination process months in advance of the election we can help find candidates very suited for this office who may have been lacking encouragement to run as well as getting to know the candidates in detail as the candidate pool grows.  

An optional attestation section helps all parties (voters, nominees, and nominators) become more cognizant of the roles and responsibilities (a.k.a. job description) of the school board down to a granular level and thereby helps improves the quality of the nominations and eventual board members.  Self-nominations are allowed.  

Nominations are not binding on running for office and will be kept confidential.

The nominator should only attest to whichever roles and responsibilities they feel comfortable attesting to.  This is a fairly exhaustive list that the board has to satisfy as a whole and it is not expected that every nominator will be able to attest to all of these statements.

Maximum 3000 characters which are approximately 600 words.  For self-nominations, this can be the same letter used in election filings if limited to 200 words.  There is no minimum number of words.
