Dear MVS community,

Please fill out this form to nominate a potential MVS Athletic Hall of Fame inductee.

This individual can be a former MVS Athlete or Coach/Support Person.

To be eligible for consideration for the MVS Athletic Hall of Fame, a student-athlete nominee:

- Has been graduated from The Miami Valley School for 10 years
- Has been an athlete, in any sport, in any era.
- Has attended MVS for at least one full year.
- Has exhibited outstanding achievement, leadership, or other contributions to MVS athletic programs.
- Has been nominated by a member of the committee, anyone at large, or themself.
- Has upheld the core values of the MVS community (while at MVS and post MVS).
- Has taken the skills from their MVS athletic experience to contribute beyond MVS.

To be eligible for consideration for the MVS Athletic Hall of Fame, a coach or support person nominee must:

- Have been a coach or support person in any sport and in any era.
- Have exhibited outstanding achievement, leadership, or other contributions to the MVS athletic programs.
- Have been nominated by a member of the committee, anyone at large, or themself.
- Be a coach or support person for at least one season; however longevity is given more weight in the selection process.
- Has upheld the core values of the MVS community (while at MVS and post MVS).
