Please complete this form to request a donation from the Burlington Township Foundation 501(c)(3).  All requests will be reviewed and voted on by the Burlington Township Foundation's Executive Board.   Donations will be considered if they comply with the Bylaws of the Burlington Township Foundation.  All requests must be submitted using this form and require at least 30 days notice.  Decisions are based on a first come, first served basis and may require documentation upon approval.   Recipient's must be located in Burlington Township, NJ.
