Report absences using this form or by calling the school attendance line at 847-398-4292, option 1, no later than 9:05am the date of the absence.
Arriving Late
Report late arrivals using this form or by calling the school attendance line at 847-398-4292, option 1, no later than 9:05am the date of the late arrival. The adult dropping the student to school must come into the office to sign the student in.. Do this by scanning the QR posted on the front window of the office to pull up the sign-in/sign-out form. Complete the form and speak to the office staff to check your student in.
Leaving Early
Please email the teacher and
westgateoffice@sd25.org to request an early pickup for your child. Please include the date and time of pick up. A guardian may authorize another person to pick up the child, but release will only be granted if the parent notifies the office in advance. This authorized pick up person must be over the age of 18 and show photo ID. The guardian or authorized pick up person must come into the office to sign the student out by scanning the QR posted on the front window of the office to pull up the sign-in/sign-out form. Complete the form and speak to the office staff to have your student called down to the office.
LEGAL NOTICE: This form is to be filled out and submitted only by the student's parent or legal guardian. In the event that a person other than the student's parent or legal guardian fills out and submits this form, AHSD25 reserves the right to take any legal action for fraudulent submission and/or take disciplinary action against any AHSD25 student(s) involved. We reserve the right to call home and verify the absence as needed.
