Non-Public schools are welcome to complete this form; however, a fee will be charged for this service. Please note even if you seeking direct student service, please have your LEA complete a request for an Independent AT Evaluation for School-Aged Students (fee-based). This request is specific to professional development/training for educators to help them provide the direct AT service to their students.
