Please fill out the following form to report any loss or damaged equipment that needs to be repaired or replaced.

Per the MOU:
"All requests by a Team and / or Team member for payment of operational expenses and /or replacement or repair costs as set forth herein shall be submitted to the Sheriff in writing within three business days after the Team is cleared from the incident, and provide the circumstances of loss, description of items and replacement costs."
