Credit On File Payment Request

This form is used to request a check for any credit on file in your Strike Force Tour account. This credit may include unclaimed prize winnings from past tournaments.

Important Notes:

  • If you'd like, credit can be redeemed online at shop.strikeforcetour.com towards any entry fees, but not in person at the events.
  • This form is the only way to request a check for your credit.
  • Please allow 7-10 business days for your request to be processed, your check to be mailed and arrive to you.
  • This request is handled by our admin team. Tournament staff cannot expedite or provide updates on the status of your request.
